Permission to Contact Form
Anyone in the MSU community experiencing relationship violence can sign the Permission to Contact (PTC) form and ask that a support person return it to MSU Safe Place. No one is required to sign the form. The person signing the form may designate how they wish to be contacted. They are not obligated to talk to or meet with MSU Safe Place staff when contacted. The PTC form is provided for survivors and for people concerned about a survivor because often people in crisis intend to follow up on referrals provided to them, but they do not always do so because of the stress they are under. The PTC form is offered to make the process of accessing support easier for survivors.
